- Prior to your treatment, all new patients must PRINT, FILL OUT, and SIGN all forms below prior to first appointment.
- Bring all forms with you to your first visit.
- An original Arbitration Form and Informed Consent Form will be given to you to sign at the time of your appointment.
- Please remember that we do not bill any business managers or insurance companies.
- Payments, in the form of Check or Cash, are expected at the time services are rendered.
- Please allow two hours for your first visit. If you have any questions please feel free to call the office.
